A Clinical Observation of the Surgical Biliary Tract Disease



Abstract
This report consists of review and analysis of 401 cases of the surgical biliary tract pati-ents who treated by surgical intervention from Jan. 1978 to Dec. 1983 for 6 years at Dept. General Surgery of Hangang Sacred Heart Hospital, Hallym College.
1) Among 401 patients, acute inflammatory diseases were 164 cases (40. 9%), chronic infl-ammatory diseases were 174 cases (37. 1%), and parasitic and malignant disease were 19 and 30 cases.
2) The sex ratio was 1 male to 1.9 female in benign groups and 1.7 male to 1 female in malignant groups. The age distribution indicated prominent incidence in 6th decade.
3) The prominent symptom and sign were pain in RUQ (67.7%) and fever and chill (58.5%).
4) The most diagnostic accuracy was abdominal C-T and ultrasonogram and P.T.C. were followed in order.
5) In 325 cases of calculous biliary tract disease, the location of stone were 137 cases (42. 2 %) in gall bladder, 83 cases (25. 5%) in common bile duct, 5 cases (1. 5%) in hepatic duct, 76 cases (23.4%) in gall bladder and common bile duct.
6) Associated biliary tract diseases were peptic ulcer (8.4%), bile peritonitis (4.2%), bili-ary cirrhosis (3.9%) and liver abscess (3.0%).
7) Various operative procedures were performed in the cases; Cholecystectomy with T--tube choledochostomy in 161 cases, cholesycteomy only in 82 cases, choledochoenterostomy in 15 cases.
8) Bile cultures disclosed Coliform bacilli (26.3%), Klebsiella, Staphylococcus, Proteus vulgaris were followed in order.
9) The residual stone was the most common specific postoperative complication (6.5%) in benign disease.
10) The mortality rate of of theis series was 0.7%.
